ABSTRACT

El diagnóstico de la infección por Clostridioides dfficile (ICD) ha aumentado en el embarazo y periparto. Cambios fisiológicos e inmunológicos normales durante el embarazo pueden incrementar el riesgo de ICD. Mujeres embarazadas con ICD tienen una mayor frecuencia de fracaso al tratamiento y una significativa morbilidad y mortalidad. El trasplante de microbiota fecal (TMF) se ha convertido en el tratamiento estándar de la ICD recurrente y refractaria. Sin embargo, existen escasos datos sobre sus resultados en mujeres embarazadas. Presentamos el caso de una mujer embarazada que se sometió con éxito a un TMF para el tratamiento de una ICD recurrente.


The diagnosis of Clostridioides dfficile infection (CDI) in pregnant and peripartum women has increased. In this scenario, there are higher rates of treatment failure and a significant maternal morbidity and mortality. Fecal microbiota transplant (FMT) has become the gold standard for the treatment of recurrent and refractory CDI however, there are few data on its results in pregnant patients. This case showed that FMT could be a therapeutic strategy in pregnant women with recurrent CDI.

ABSTRACT

Clostridioides difficile infection (CDI) is a major public health problem and responsible for significant morbidity and mortality. Eighty percent of CDIs occur in adults older than 65 years of age due to a decreased gastrointestinal microbial diversity, immunosenescence and frailty. Thus, the most reported risk factor for recurrent CDI is older age since nearly 60% of cases occur in individuals aged ≥ 65 years. Fecal microbiota transplantation (FMT) is a highly cost-effective alternative to antibiotic treatment for patients with recurrent CDI. We report a 75-year-old male with recurrent CDI, who received a FMT after several unsuccessful antimicrobial treatments. He had a satisfactory evolution after the procedure and remained without diarrhea during the ensuing five months.

ABSTRACT

Anemia is the most common extraintestinal manifestation of inflammatory bowel disease (IBD). Although there are several causes of anemia in IBD, the two most frequent etiologies are iron deficiency anemia and anemia of chronic disease. Despite the high prevalence of anemia in IBD and its significant impact on patient's quality of life, this complication is still underdiagnosed and undertreated by providers. Active screening for anemia, structured assessment, comprehensive management, and multidisciplinary collaboration are needed in IBD patients. The cornerstone of anemia management depends on the underlying etiology along with normalization of inflammatory activity. Although, oral iron is effective for the treatment of mild iron deficiency-related anemia, intravenous iron formulations have a good safety profile and can be used as first-line therapy in patients with active IBD, severe anemia and previous intolerance prior to oral iron. After proper treatment of anemia, careful monitoring is necessary to prevent its recurrence. Herein, we discuss the etiology, screening, diagnosis, therapy selection, and follow-up for anemia in IBD.

ABSTRACT

Biological therapy dramatically changed the management of Ulcerative Colitis (UC). However, a significant number of these patients fail to respond or have secondary loss of response to this strategy. In this clinical situation, the options include intensification of anti-TNF therapy, the use of a second anti-TNF or being switched to another drug class. Among the later, tofacitinib, an oral small molecule directed against the JAK/STAT pathway, is safe and effective in inducing and maintaining remission in patients with moderate-severe UC. We report two patients with UC refractory to conventional treatment and biological therapy, who responded successfully to the use of tofacitinib.

ABSTRACT In patients with ulcerative colitis refractory to medical therapy, total proctocolectomy and posterior ileal-anal pouch anastomosis is the standard surgical therapy. One of the possible complications is pouchitis. Depending on the duration of the symptoms, it can be classified as acute, recurrent, or chronic. The latter, according to the response to therapy, can be defined as antibiotic-dependent or refractory. The treatment of pouchitis is based on the use of antibiotics and probiotics. Thiopurine and biological therapy have been suggested in patients with refractory pouchitis. Special care should be taken in the endoscopic surveillance of these patients, especially if they present risk factors such as dysplasia or previous colorectal cancer, primary sclerosing cholangitis or ulcerative colitis for more than 10 years.


RESUMO Em pacientes com colite ulcerativa refratária à terapia médica, a proctocolectomia total e anastomose de bolsa ileal-anal posterior é a terapia cirúrgica padrão. Uma das possíveis complicações é a pouchite. Dependendo da duração dos sintomas, pode ser classificado como aguda, recorrente ou crônica. Esta última, de acordo com a resposta à terapia, pode ser definida como dependente de antibióticos ou refratária a eles. O tratamento da pouchite baseia-se no uso de antibióticos e probióticos. A thiopurina e a terapia biológica têm sido sugeridas em pacientes com pouchite refratária. Um cuidado especial deve ser tomado na vigilância endoscópica desses pacientes, especialmente se apresentarem fatores de risco, como displasia ou câncer colorretal anterior, colangite esclerosante primária ou colite ulcerativa por mais de 10 anos.

ABSTRACT

Background: Colorectal cancer (CRC) is the third most prevalent cancer in the world and is the second cause of cancer death. Positron emission tomography/computed tomography (PET/CT) using 18F-FDG is used for its staging and follow up. Aim: To assess the occurrence of synchronous colonic and extracolonic tumors detected with contrast-enhanced F18-FDG PET/CT (PET/CTc) in patients with a recently diagnosed CRC. Material and Methods: PET/CTc of 210patients aged 16-91, years (63% men) with a recently diagnosed CRC were reviewed. PET/CTc with incidental findings, not expected to be due to CRC, were followed (laboratory, imaging and pathology) searching for synchronous tumors. Results: Ten patients (4,7%) had a second synchronous CRC. Only 70% of synchronous CRC were accessible to colonoscopy, due mainly to incomplete procedures for stenotic tumors. Extracolonic synchronous neoplasms were detected in 12 patients (5,7%), namely lung cancer in three, renal cell carcinoma in two, non-Hodgkin lymphoma in two, pancreatic cancer in one, breast cancer in one, hepatocellular carcinoma in one, bladder cancer in one and thyroid cancer in one. Conclusions: Ten percent of patients with a recently diagnosed CRC had a synchronic neoplasm detected at staging using PET/CTc.

SUMMARY: Ultrasound is one of the most used tools in physiotherapy, its LIPUS modality allows to treat musculoskeletal injuries, but like the conventional ultrasound it is contraindicated its application in areas close to growth cartilage by a possible closure of this one. In the literature there is no conclusive evidence of this contraindication, so the objective of the present study is to evaluate the effects of doses equivalent to underwater LIPUS on the histomorphometry of the proximal tibial epiphysis growth plate in an animal model. LIPUS was applied in underwater mode in the right pelvic limb to 8 mice in p 15 for 4 days, 2 times a day. On completion of 8 sessions the histology of the stimulated limb growth plate was sacrificed and the histology. When evaluating the histomorphometry of the plaque, a significant increase in the thickness and area of the proliferative zone stimulated with LIPUS (p = 0.048) was observed. In the hypertrophic area were observed no significant differences in thickness or areas, but in the hypertrophic area there were significant changes in the histological organization, observing a significant increase of the entire columns in the stimulated plaque (p = 0.04). LIPUS in underwater modality modifies the histomorphometry of the proximal epiphyseal PC of the tibia of rodents in the late postnatal stage, promoting proliferation and columnar organization in the proliferative and hypertrophic zone, respectively.


RESUMEN: El ultrasonido es una de las herramientas más utilizadas en fisioterapia, su modalidad LIPUS permite tratar lesiones músculo-esqueléticas, pero al igual que el ultrasonido convencional está contraindicada su aplicación en zonas próximas a cartílago de crecimiento por un posible cierre de éste. En la literatura no existe evidencia contundente de esta contraindicación, por lo cual el objetivo del presente estudio fue evaluar los efectos de dosis equivalentes a LIPUS subacuático sobre la histomorfometría de la placa de crecimiento de la epífisis proximal de tibia en un modelo animal. Metodología. Se aplicó LIPUS en modalidad subacuática en el miembro pélvico derecho a 8 ratones en p 15 por 4 días, 2 veces al día. Al cumplir 8 sesiones se sacrificaron y se evaluó la histología de la placa de crecimiento del miembro estimulado. Al evaluar la histomorfometría de la placa se observó un aumento significativo en el espesor y el área de la zona proliferiva estimulada con LIPUS (p=0.048). En la zona hipertrófica no se observaron diferencias significativas en el espesor ni en las áreas, pero si existieron cambios significativos en la organización histológica de la zona, observando un aumento significativo de las columnas completas en la placa estimulada (p=0,04). El LIPUS en modalidad subacuática modifica la histomorfometría de la PC epifisiaria proximal de la tibia de roedores en el etapa postnatal tardía, promoviendo la proliferación y la organización columnar en la zona proliferativa e hipertrófica, respectivamente.

Introducción: La reforma de la salud en Chile incluyó una Política Nacional de Medicamentos que posiciona al profesional químico farmacéutico en la tarea de contribuir a lograr los objetivos terapéuticos y de uso racional de los medicamentos. La investigación tuvo el objetivo de explorar las percepciones de químico farmacéuticos respecto a la implementación de los cambios introducidos por esta Política. Material y Método: Se condujo un estudio cualitativo y exploratorio en base a entrevistas individuales semi-estructuradas en profundidad a profesionales químico farmacéuticos de la red asistencial de la Región Metropolitana durante el período marzo-agosto de 2010. Se seleccionó una muestra por conveniencia de 10 profesionales de diferentes niveles de atención de la red asistencial de la Región. El análisis de las entrevistas se realizó mediante la técnica de análisis de contenido. Resultados: Para los entrevistados, la Política Nacional de Medicamentos ha permitido asignar un rol importante al farmacéutico y a la farmacia dentro de las distintas instancias de la red asistencial. No obstante, las principales dificultades en su implementación han estado determinadas por la ausencia de condiciones organizacionales y estructurales de las instituciones relacionadas con la implementación de la reforma de la salud.Discusión: La implementación de la Política Nacional de Medicamentos ha permitido mejorar algunos procesos de la gestión farmacéutica. Sin embargo; las brechas entre el diseño y la implementación de la reforma de la salud ha obstaculizado su implementación.


Introduction: Health reform in Chile included a National Medication Policy that positioned pharmacists as contributors to achieving therapeutic goals, and as promoters of the rational use of medications. The objectiveof the research was to explore the perceptions of pharmacists with respect to the implementation of changes introduced by the National Medication Policy. Materials and Methods: A qualitative and exploratory study, based on individual, semi-structured, in-depth interviews with pharmacists in the Metropolitan health care network, was carried out during March- August of 2010. A convenience sample of 10 professionals was selected from different health care levels within the Metropolitan network. The analysis of the interviews was carried out using content analysis.Results: For the interviewees, the National Medication Policy has given pharmacists and pharmacies an important role in different sectors of the health care network. Nevertheless, the principal difficulties in its implementation have been determined by the absence of different organizational and structural conditions in the institutions related to the implementation of the health reforms.Discussion: The implementation of the National Medication Policy has improved some processes of pharmaceutical management. Nevertheless, the gaps between the design and implementation of health reform have hampered its implementation.
